Provider Services need Home Care Coordinators to work within the Thurrock Care at Home Team based in Thurrock, where domiciliary care and re-ablement are provided. We cater for vulnerable adults with differing needs and those people require extra support with personal care duties.Our homecare staff work in a way to promote personalisation and re-ablement while assisting in achieving the delivery of outcomes for residents within their own home. You will be responsible in supporting the existing team by supervising a team of patch based carers, assist with scheduling care calls using CM2000, you will also be required to go into clients homes to undertake Quality Assurance visits, create client support plans, and if necessary support the clients with doing hands on care. You will be expected to promote our core values that include individuality, independence, choice, dignity, respect, rights, confidentiality and fulfilment, which guarantees users of the service, receive a high standard, quality service.The type of person who will be suitable for this role will need patience, understanding, flexibility, be committed and have a genuine caring nature and be able to demonstrate the core values of the service. You will be required to undertake a full induction and training plan in line with the Care Certificate.Ability to work unsociable hours is essential.
