Hours: 37 hours per week to cover shift patterns.  Shifts 06:00 to 14:30 or 14:30 to 23:00 7 days a weekProvider Services need Home Care Coordinators to work within the Thurrock Care at Home Team based in Thurrock, where domiciliary care and re-ablement are provided. We cater for vulnerable adults with differing needs and those people require extra support with personal care duties.Our homecare staff work in a way to promote personalisation and reablement while assisting in achieving the delivery of outcomes for residents within their own home.You will be responsible for:Supporting the existing team by supervising a team of patch based carersAssisting with scheduling care calls using CM2000,Going into clients homes to undertake Quality Assurance visitsCreating client support plansSupporting clients with doing hands on care.You will be expected to promote our core values which includes:IndividualityIndependenceChoiceDignityRespectRightConfidentiality and fulfilment, which guarantees users of the service, receive a high standard, quality service.The ideal candidate for this role will need to be:PatientUnderstandingFlexibleCommittedGenuine caring natureYou will be required to undertake a full induction and training plan in line with the Care Certificate.Ability to work unsociable hours is essential.  The ability to get around the borough in a timely fashion is essential – mileage is paid at 40 pence per mile and you will receive a monthly payment to support the use of your own vehicle.Shift allowance of 10% is paid for shifts worked before 8.00 am and after 8.00 pm.  For an informal discussion about these posts, please contact Karen Hogger on 01375 652997 or Kallie Morgan on 01375 652984.A DBS at Enhanced level will be required for this post.Our vision – An ambitious and collaborative community which is proud of its heritage and excited by its diverse opportunities and future.Thurrock is an exciting place to live and work.
