Job Title: Mental Health Support Worker  Responsible to:Line Management: TBCHours:15 hours per week  Grade: Band C  Location:                   Base: TBC                                    Works across Manor, Castle, Woodthorpe    Overall Purpose of the Role:                             To work with people with mental health issues living in the community. To support them to become more socially, economically and physically active with the purpose of reducing social isolation. Key Responsibilities To work with identified individuals to assess their needs and develop a support plan working towards maintaining independent living and reducing social isolation To carry an individual caseload and maintain and update support plans to ensure a quality service is maintained for each client To carry out home visits and initial assessments and complete case file profiles and update support plans in line with contract legislation. To work with all of the MCDT services and wellbeing initiatives to ensure the best outcomes are met for each individual.To link with the existing MCDT Spring Board cafe.To develop relationships with other service organisations and agencies and advocate on behalf of the client to coordinate services required.To support existing mental health and wellbeing groups to develop and grow.To engage with potential new members and support them to access the existing provision.To promote services that already exist in the local community.To support the development and delivery of health promotion events which raise the profile of mental health and wellbeing.To support and manage volunteers when needed.To maintain records for monitoring purposes on a regular basisTo produce reports to a high standard as and when required.Work within Manor & Castle Development Trust policies regarding confidentiality and record keeping.Attend regular supervision, personal development and staff meetings.Understand and apply Health & Safety requirements relating to self and clients.To undertake any training necessary to the post.To undertake all the above in line with Manor and Castle Development Trust`s aimsTo undertake any other duties within reason, negotiated between post holder and line manager.  Post Title: Mental Health Support Worker Person Specification Essential DesirableSkills/Knowledge  Local knowledge of the Manor, Castle and Woodthorpe areas.  Knowledge of what factors effect mental health and wellbeing.  Knowledge of the causes of health inequalities  Understanding of the barriers people face to improving their mental health and wellbeing.  Knowledge of effective behaviour change and health promotion models  Understanding of the principles of community development.  Good at listening to people  Able to identify, assess and manage risks  Knowledge and/or understanding of safeguarding   Supportive and encouraging to people in difficult situations without making a judgement about them.  Able to motivate others  Knowledge and understanding of confidentiality  Experience and Qualifications  Ability to communicate effectively both verbally and in writing, with a range of people and organisations  Experience of working with community groups in some capacity, or experience of community development work with knowledge about group facilitation/training methods. (voluntary or paid)  Experience of working with people with mental health issues  Experience of supporting people to access health services or experience of supporting people to participate in health enhancing activity.  Experience of recruiting and managing volunteers  Experience of delivering community based activity.  To be computer literate and undertake basic admin duties such as report and letter writing, filing and record keeping  Ability to work independently and as part of a team  Experience of advocacy/support work  Experience of working with vulnerable people(voluntary or paid)  Personal Qualities  Enthusiastic  Committed to learning  Empathy  Non-judgemental  Flexible approach, including working at weekends and evenings  Willing to work with people from all walks of life, different ages, sex, ethnicity, religion, different socio-economic groups etc    View full job description
