INTERVIEWS WILL BE HELD ON 1ST NOVEMBER 2017Occupational Therapist (Band 6)Service/Ward/DepartmentNorth Division-Adult In-patients-BuryBoroughBuryBand &salary scaleBand 6 AfCHours of work37.5- DaysPermanent/Fixed termPermanentService/Ward/Department Summary:The Irwell Unit at Fairfield General hospital has two mixed sex acute mental health wards providing services for adults of working age in the Borough of Bury.Role Summary:The successful candidate will be able to work effectively as an Occupational Therapist have excellent communication skills, welcome a challenge and have a real desire to make a difference to the care we provide to the adult age group inclusive of evolving models of care and modernisation of mental health services.Key Responsibilities of the Post:? To be responsible for the assessment treatment and evaluation of patients referred specifically for OT intervention from the Inpatient Services.? To work as a member of the multidisciplinary team on the wards.? To assess OT need and plan therapeutic OT interventions using the Model of Human Occupation.? To liaise, plan and co-ordinate with other professional agencies including wards, discharge coordinator, CMHT, Housing, Third Sector Bodies in order to ensure continuity and quality of service.? To participate in discharge planning meetings, care plan reviews, CPA meetings and informal discussions concerning patients.? To be responsible for developing screening processes to ensure appropriate referrals in line with O.T. Acute Care Forum? To be responsible for managing a caseload of clients identified through screening processes.? To write reports using MOHO outcome measures and to ensure up-to-date and accurate records are maintained regarding all patient contact.? As a member of the MDT to lead on the co-ordinated discharge of patients, providing a ‘seamless service’ for patients transferring from inpatient to community care.2? To work with in-patients in their own homes in the community or hospital based facilities where relevant.Key Requirements of the Person Specification:The ideal candidate should be enthusiastic and flexible in approach and must hold a diploma or degree in Occupational Therapy and be registered with HSPCYou should have a knowledge of MOHO and its’ assessment tools and possess a good understanding of the occupational context of acute care and its’ application to assessment and treatmentYou will have a good understanding of Mental Illness.You must have evidence of Continuing Professional Development (CPD) and a sound understanding of current government policies in relation to Acute Care including National Guidance, MHA , Risk Assessment, Risk Management processes and CPA processes and a good working knowledge of assessment, care planning, implementation and evaluation processes.INTERVIEWS WILL BE HELD ON 1ST NOVEMBER 2017This advert closes on Wednesday 18 Oct 2017 View full job description
