Connect2kent are currently recruiting for a Senior Practitioner based in Sittingbourne .Main duties and responsibilities:*Manage case work within a designated GP aligned team or to a designated group of professional staff in the ACT, providing supervision (including supervised practice sessions), and ensuring team members deliver effective, safe and high quality assessments &reviews to their clients and that clients receive proportionate holistic assessments (including where appropriate self-assessments and carers assessments) and are able to engage appropriately and identify needs and outcomes. This will include consideration / actions relating to the Mental Capacity Act and/ or Safeguarding.*Responsible for wider professional practice / clinical oversight across the ACT utilising professional expertise to support OT, Social Work or Nurse qualified staff to ensure professional practice is supported, and competencies maintained. *Lead on an area of work across the Area including the development of new initiatives (e.g. extra care sheltered housing panels, dementia care, carers, transition, equipment and adaptation authorisations.Housing Adaptations and Joint Funding Panels) through representation on working groups or multi-agency for a, training courses etc. and act as a lead for identified pieces of work that will shape current and new ways of working.*Undertake the Designated Senior Officer role in safeguarding and quality of care issues in line with directorate policy and practice requirements, taking forward appropriate actions and communication with supervisor / line manager/ other directorate officers. Oversee of all complex / safeguarding cases in the designated GP aligned team within the ACT *Manage a caseload made up of highly complex / safeguarding cases that require a high level of expertise and specialist input within the parameters of the Directorate's agreed policies and procedures and promote enablement to ensure access to this service with a view to promoting independence and increasing levels of control a person has over their life.*Be accountable for own performance and that of the designated team regarding adherence to the Directorate's policies relating to Assessment, Support planning, promoting independence reviews, record keeping and data protection. Maintain professional portfolio of practice evidence to support professional development and management of increasing complexity. *Monitor and evaluate the designated GP aligned team's performance as provided in trackers and taking appropriate action to maintain high practice standards. Ensure a weekly review with the Team Manager and produce case reports and performance information as required to the Service Manager / Assistant Director and wider Area Management Team. Take shared responsibility for the daily 'wash up' sessions and use of the improvement cycle.*Support the introduction of 'extended working hours' practice through participation in the senior stand by rota, supporting registered and unregistered staff working on acute hospital sites at weekends, public and bank holidays to ensure greater access to services.*Participate in the Referral Management Service rota as the senior oversight of cases for the designated period and ensure clients receive appropriate advice, guidance and support.*Support the Team Manager in the recruitment and induction of new staff and in managing the development of the team's knowledge base and expertise as well as supporting the day to day management of the team to ensure services are delivered and meet the standards required.*Initiate and progress integrated working with health providers (GP's, community nursing, intermediate care, mental health services, acute health providers), the private and voluntary sector local housing providers, district and borough councils (as appropriate eg Joint Management Groups) and other statutory agencies.*Provide professional supervision for students on practice placements with the team, planning a range of work experience, supporting their development, monitoring progress and supporting the embedding of the student's learning in practice.*Undertake the role of Care Supervisor in delivery of any required emergency response in accordance with business continuity plans. if you have the necessary skills and qualifications we look forward to hearing from you Commercial Services Kent/Trading Ltd is an equal opportunities Employment Agency &Business and positively encourages applications from all suitably qualified and eligible candidates.
