Salary starts at £16,523 to £17,000 per annum pro rata rising to £18,500 pro rata through performance.Whose life have you changed today?Home Group's Care and Support services change and save thousands of vulnerable people's lives every single day.  Within our inspirational Care and Support team, each individual colleague truly makes a real difference, working together to support our customers to make aspirational and life-long changes.  Join us and see what an amazing difference you can make at work every single day!We have a fantastic opportunity for an enthusiastic and caring Support Worker like you (known internally by us as Support Worker – Level 2) to join our service which provides support within the local community to promote independence.The role includes supporting our customers to develop life and social skills so they can move on successfully.  You will be supporting the Support Coordinators to ensure that our customer’s get the most out of there stay with us by facilitating customer involvement activities and group work. Some of the excellent work we have done includes gardening projects, cooking groups and wellbeing awareness sessions.   We would love you to bring your creativity to the role. The service consists of 3 elements, this entails delivering floating support to customers in their own home, drop in support in one of the many venues that we work out of in the Derwentside Area and group work within the community.An experienced Support Worker, you’ll help change lives for the better by assisting in delivering identified tasks based upon customers’ individual needs and aspirations, as identified within their personal support plans.  You’ll liaise with external agencies and work closely with our Support coordinators too in assessing and engaging support for our customers.  Together you will really make a difference, empowering our customers to develop the independent living skills they need to successfully move on with a positive future.It’s really important to us that you have time to what you are really care about; changing customers lives for the better.  So to free up your time, you’ll be given the latest IT kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system which means that you don’t have to be in the office as you can complete support plans on the go. A bit about us.We’re Home Group one of the UK’s biggest providers of high quality housing and care and support services.  We’re also one of the largest developers of new homes and providers of houses for affordable rent. A bit about youWe want someone like you, with the passion and motivation for helping vulnerable people make positive and lasting changes to their lives.  To flourish in the role, you’ll have a practical understanding of safeguarding, risk assessments, confidentiality and support plans.
