 Job DescriptionSection/Service:Independent Living Service  Department: Working Age AdultsJob Title: Support Worker   The Independent Living ServiceDescription of service; To provide Person-centred Support, that is appropriate for the individual, reflecting their uniqueness, supporting them to be fully involved in their community and to ensure that they are afforded the same rights and responsibilities as any other citizen. Supporting each person within the principles and philosophy of ordinary life. Primary Purpose of RoleTo contribute to the overall running of the service supporting the registered manager in ensuring CQC compliance. To provide support that is safe, person centred and respects the human rights of the people we support.This role involves working across various locations in the community, including our supported living schemes, to cover regular staffs’ annual leave and sickness.  To ensure that the people we support are able to live their lives in ways that makes sense to them and to ensure that those we support are well connected in their communities, enjoy and achieve, stay safe, make positive contributions, and remain healthy. To ensure that the people supported are at the core of everything we do, that they are treated with respect, and that their dignity is protected, and to ensure they are free to express themselves and have their views listened to and acted upon. Main Duties and Responsibilities ?     Supporting people to become valued members of their community, to develop friendships and relationships, have a life that makes sense to the individual. ?     People are supported have a full and active life both in their home and in the community, are able to follow their interests and hobbies, and have the chance to enjoy new experiences. Viewing “difficulties” as opportunities, working with a can do attitude.?     Developing daily living skills – thus ensuring individuals develop independence – breaking tasks down into small manageable steps.?     Supporting this for all individuals, but taking the lead where acting as a key worker.?     Being accountable for this work to the people we support, the Manager and the whole team, ensuring positive outcomes for those we support. ?     Supporting people to have a full life in which they are safe, working in partnership with external professionals contributing to positive behavioural support plans.?     Ensuring that individuals have as wide a circle of support network as possible.?     Supporting individuals to remain healthy and safe, supporting them within a positive risk taking environment.?     Where appropriate ensuring that those we support have access to and are supported to claim all their entitlements to welfare benefits.?     To work with the individual, appointee, family/advocate to ensure the individual is well supported to make decisions as to how they wish to use their money and able to budget for their living expenses e.g. budget for holidays, clothes shopping.?     To ensure that those we support have access to good health care, screening, annual health checks and acts as advocate when needed to ensure fair and equal access to this. To support individuals with all aspects of their health.?     To ensure that those we support have full and active lives – with regards to support to take part in leisure and other active pastimes, where required motivating and encouraging – working in partnership with others to work towards this goal.
